Ordering your NHS prescriptions from Jays Pharmacy is simple. Just download and fill in the order form below with your details and medications, then post it to us or drop it in at the Pharmacy.

We can collect your prescription from any Surgery locally, or alternatively if your surgery is EPS ready we can access your prescription from any Surgery nationwide.
Your prescription should then be ready within 48 hours - you can call into the Pharmacy to collect it or we can we can deliver it to you within reasonable distance.
Here's how it works:
• Download & complete our Repeat Prescription Management Sign up Form
• Post the form to your surgery or drop it in at the surgery
• Please notify us if there are controlled drug prescriptions to be collected
• We’ll make sure that your medication is ready each time you need it
• You can collect your prescription from the Pharmacy or we can arrange free delivery* to your door.
Everything is done onsite and all patient information is safe, secure and confidential. We source from a wide range of suppliers to ensure our customers’ needs and preferences are satisfied.
